The row and column counts have to match the original image's grid exactly. After entering the numbers, check on the canvas whether the red lines sit on the image's own boundaries, and drag them into place if they don't.
Yes. For Instagram's three-column grid, set columns to 3 and rows to however many lines you want. Each cell comes out close to square. Split, then upload the pieces in numbered order and the grid assembles itself.
Cells are saved as PNG. File names are generated as cell_r{row}_c{column}.png — for example, cell_r1_c2.png.
